Facial
Botox Cosmetic in New Jersey & New York City
Natural-Looking Botox Treatments by Board Certified Plastic Surgeon Dr. Joseph Racanelli.
Facial expressions are an important part of how we communicate, but over time repetitive muscle movement can create visible lines and wrinkles. Botox® Cosmetic is one of the most effective non-surgical treatments available to soften these dynamic wrinkles and create a refreshed, more youthful appearance.
Botox treatments are performed by board-certified plastic surgeon Dr. Joseph A. Racanelli. His approach focuses on natural-looking results that soften wrinkles while preserving facial expression. The goal is not to make patients look frozen or overtreated, but rather refreshed, rested, and confident.
Patients travel to our offices from throughout New Jersey and New York, including Morristown, Summit, Short Hills, Chatham, Madison, Livingston, Montclair, Jersey City, Hoboken, Manhattan, and surrounding communities.

What is Botox?
Botox Cosmetic is an FDA-approved neuromodulator that temporarily relaxes specific facial muscles responsible for creating expression lines. By reducing muscle activity, Botox softens existing wrinkles and helps prevent deeper lines from developing over time.
Botox has been extensively studied and has been safely used for both cosmetic and medical purposes for decades.
How Does Botox work?
When Botox is injected into targeted muscles, it temporarily blocks the nerve signals responsible for muscle contraction. As these muscles relax, the overlying skin appears smoother and less wrinkled.
The treatment does not permanently alter the muscles or skin. Over time, muscle activity gradually returns and maintenance treatments are recommended to preserve results.

Areas Commonly Treated With Botox
Forehead Lines
Horizontal forehead lines often become more noticeable with age. Botox can soften these lines while maintaining natural movement and expression.
Frown Lines (Glabellar Lines)
Often referred to as the "11s," these vertical lines between the eyebrows can create an angry or tired appearance. Botox is highly effective at relaxing the muscles responsible for these wrinkles.
Crow's Feet
Fine lines around the eyes often develop from smiling and squinting. Botox can soften these lines while preserving a natural smile.
Bunny Lines
Small wrinkles along the bridge of the nose can be effectively treated with Botox.
Neck Bands
Prominent platysmal bands of the neck can often be softened with Botox treatment.
Dr. Racanelli's Approach to Natural-Looking Botox
One of the most common concerns I hear from new patients is that they're afraid of looking frozen, and honestly, that fear is valid. I've seen it done poorly. The frozen look almost always comes from over-treating the forehead without accounting for how it affects brow position, or from injecting the same number of units into every patient regardless of their muscle strength.
My approach starts with watching you move your face before I inject anything. I ask you to raise your brows, squint, frown. I'm mapping your specific muscle recruitment patterns, which muscles dominate, where asymmetries exist, how your skin sits at rest versus in motion. That assessment takes a few minutes and it's what separates a result that looks natural from one that looks treated.
I also treat conservatively on the first visit. It's always easier to add than to take away, and every treatment includes a complimentary two-week follow-up where we reassess together. If there's still movement you'd like softened, we address it then at no additional cost. This two-step process consistently produces better outcomes than trying to get everything perfect in a single session.
Every Botox treatment at our practice is performed personally by Dr. Racanelli. We do not have nurse injectors on staff. This matters more than most patients realize, in many practices, you book with the surgeon but are treated by a delegated provider. Here, the person who assessed your facial movement in the consultation is the same person holding the syringe.
Why See a Board-Certified Plastic Surgeon for Botox?
Botox is available almost everywhere, medspas, dermatology offices, even dental practices. So why does it matter who injects you?
As a board-certified plastic surgeon, my training in facial anatomy is fundamentally different from that of a nurse injector or general practitioner. I understand the layered structure of the face, the muscles, the fat compartments, the fascial planes, the nerves, because I operate on them. That anatomical knowledge directly informs where I place each injection, how deep I go, and how much product I use.
This matters practically. The muscle that creates crow's feet sits differently depending on the patient's orbital anatomy. The glabellar complex, the muscles responsible for frown lines, involves up to five separate muscles, and treating them without understanding their individual contributions can shift rather than smooth the wrinkle. These are the details that produce consistently natural results versus outcomes that look off in ways patients can't quite name.
It also matters for safety. Complications from Botox, lid ptosis, brow asymmetry, spread to unintended areas, are rare when performed correctly, but managing them requires a deep understanding of facial anatomy. I have that foundation.
Botox may be a simple treatment, but simple is not the same as easy to do well.
Botox VS. Dermal Fillers
Many patients are unsure whether Botox or filler is the appropriate treatment.
Botox
Dermal Fillers
During your consultation, Dr. Racanelli can determine which treatment, or combination of treatments, will best achieve your goals.
When Will I See My Results?
Day 1-3
Minimal visible change.
Day 4-7
Early softening of facial movement begins.
Day 10-14
Full results become visible.
Month 3-4
Gradual return of muscle activity.
Maintenance
Most patients return every 3-4 months to maintain their results.
Am I a Good Candidate for Botox?
Most healthy adults bothered by dynamic wrinkles, lines that appear or deepen with facial movement, are good candidates for Botox. The treatment works best when wrinkles are primarily movement-driven rather than caused by volume loss or skin laxity, which are better addressed with fillers or surgical options.
Botox may not be appropriate if you are pregnant or breastfeeding, have a neuromuscular condition such as ALS, myasthenia gravis, or Lambert-Eaton syndrome, or are taking certain medications that affect neuromuscular function. During your consultation, I review your full medical history to ensure Botox is safe and appropriate for you.
Younger patients in their 20s and early 30s often ask about preventative Botox, using small amounts to slow the formation of deeper lines before they become etched into the skin at rest. This is a legitimate and increasingly common approach, and one I discuss with patients on an individual basis based on their anatomy and how their skin is aging.
Frequently Asked Questions
How much does Botox cost in New Jersey?
We price Botox by treatment area, not by the unit. This reflects how Dr. Racanelli works: he assesses your muscle movement during the consultation and tailors the exact number of units to your anatomy and goals, rather than using a fixed formula. Pricing by area means you know your cost upfront without it varying based on how many units were used. During your consultation, Dr. Racanelli will outline exactly which areas he recommends treating and the total cost before anything is scheduled.
Does Botox hurt?
Most patients describe Botox injections as a brief pinching sensation. The procedure is quick and generally very well tolerated.
Will Botox make me look frozen?
Not with the right approach. The frozen look comes from over-treatment, too many units applied without accounting for the patient's individual muscle dynamics and brow position. My goal is for you to look like yourself, only more rested. Most patients find that people notice they look refreshed without being able to identify why.
How long does Botox last?
Most patients enjoy results for approximately 3-4 months.
Can Botox prevent wrinkles?
Yes. By reducing repetitive muscle movement, Botox may help slow the formation of deeper expression lines.
Is there downtime after Botox?
Botox requires minimal downtime. You can return to desk work and normal daily activities immediately. For the first 4 hours after treatment, avoid lying flat and do not massage or apply pressure to the treated areas. Avoid strenuous exercise, saunas, and anything that significantly raises your heart rate or body temperature for 24 hours. These precautions help ensure the product stays localized to the intended muscles.
How many units of Botox will I need?
The number of units varies based on muscle strength, facial anatomy, and treatment goals. Every treatment plan is customized.
Can I exercise after Botox?
Light activity is fine immediately. Avoid strenuous exercise, running, heavy lifting, hot yoga, and similar, for 24 hours post-treatment. After that, there are no restrictions.
What if I want more correction after my treatment?
Every Botox treatment at our practice includes a complimentary two-week follow-up. At that appointment, Dr. Racanelli reassesses your movement and, if there are areas where you'd like more softening, additional product is included at no extra charge. This is built into every first visit, it's how we ensure the result is exactly right for you without over-treating on day one.
Why Choose Dr. Joseph Racanelli For Botox?
Patients choose Dr. Racanelli for Botox because his background as a board-certified plastic surgeon brings a level of anatomical precision that most injectors don't have. Treatments are always performed by Dr. Racanelli personally, not delegated to staff. His practice is intentionally small, which means consultations are unhurried and treatment plans are genuinely individualized. He sees patients from across New Jersey and New York City, many of whom have been with him for years and return every few months for maintenance treatments.
Dr. Racanelli performs every Botox injection himself, no nurse injectors, no delegation. In a market where Botox is available on nearly every corner, often administered by someone with minimal training, that distinction is the difference between a result that looks natural and one that doesn't.
